Evoke™ Strips — Important Safety Information

Compounded drugs are permitted to be prescribed under federal law, but are not FDA-approved and do not undergo FDA safety, effectiveness, or manufacturing review. Your EOS Health-affiliated provider may recommend this medication based on your individual medical evaluation.
Evoke™ Strips contain both sildenafil 55 mg and tadalafil 22 mg as a compounded drug to treat erectile dysfunction. Your EOS Health-affiliated provider may prescribe Evoke™ Strips based on your medical evaluation.
Important Safety Information for Sildenafil
Who should not use sildenafil?
Do not use sildenafil if:
- You are taking a class of medications called nitric oxide donors, including isosorbide dinitrate, isosorbide mononitrate, or nitroglycerine. Combining sildenafil with nitrates can cause dangerously low blood pressure.
- You have a known allergy to sildenafil.
- You are taking a class of medications called soluble guanylate cyclase (GC) stimulators, such as riociguat.
How should I take Evoke™ Strips?
Place the lozenge under your tongue and let it dissolve. Take at least 15 minutes before sexual activity. Do not take more than once per 24 hours or within 24 hours of other ED medications. Avoid excessive alcohol.
What should I tell my EOS Health-affiliated provider?
Tell your provider about all medications you take. Watch for interactions with nitrates, alpha-blockers, antihypertensives, CYP3A4 inhibitors (e.g., ritonavir, ketoconazole), and alcohol (>5 units).
Share your full medical history, especially: cardiovascular risk, chest pain requiring nitroglycerin, priapism predisposition, BPH on alpha-blockers, ventricular outflow obstruction, MI/stroke/arrhythmia history, low or high BP, unstable angina, NAION, bleeding disorders, kidney/liver disease, pulmonary hypertension, retinitis pigmentosa.
Most serious side effects
If experiencing a medical emergency, call 911.
- Prolonged erections — seek emergency treatment for erection lasting >4 hours.
- Sudden vision loss or hearing loss — stop medication and seek medical attention.
- Severely low blood pressure — especially when combined with nitrates, alpha-blockers, or ritonavir.
- Cardiac risk of sexual activity.
- Combination with other PDE-5 inhibitors — do not exceed once per 24 hours.
- Sildenafil does not protect against STIs.

- Headache, flushing, indigestion, abnormal vision, nasal congestion, back pain, muscle pain, nausea, dizziness, rash.
Important Safety Information for Tadalafil
Who should not use tadalafil?
Do not use tadalafil if you take nitrates or guanylate cyclase stimulators, or if you have a known allergy.
Most serious side effects
- Cardiac risk of sexual activity.
- Prolonged erections >4 hours — emergency treatment required.
- Sudden vision or hearing loss.
- Severe BP drop with alpha-blockers or BP-lowering drugs.
- Kidney or liver impairment — dose adjustments may be required.
- Limit alcohol to <5 drinks.
- Tadalafil does not protect against STIs.
Most common side effects
- Headache, dizziness, indigestion, back pain, muscle pain, nasal congestion, flushing, pain in limb.
